Create a new image in Photoshop the size you want it. (I used 800 x 800)
Find the center of the document (Go to the View Menu -> New Guide put a guide at 50% vertical, you will have to do this again to put a guide at 50% horizontal)
Using your ellipse tool with Paths selected not Shape Layers draw a circle from the center hold down alt and shift (This will be the outside of your gear)
Now again from the center use your Rectangle tool this time only hold alt draw a rectangle a little taller then your circle this will be a tooth on the gear.
Using your path selection tool select the rectangle you just created
